Care4Today® Connect

• Care4Today® Connect is intended to be used as a helpful medication, appointment, and activity reminder tool. It can also include condition related educational content. It is not intended to be relied upon as a primary method for scheduling when to take your medication or determining what medication to take. You are responsible for ensuring that you take your medication at the correct times and dosage. Always consult your doctor regarding when to take your medications.
• Neither the App nor any “Connections” established through Care4Today® Connect is intended to be medical advice or a substitute for professional medical advice, counseling, evaluation, diagnosis, or treatment. You should always seek the advice of your physician or other qualified healthcare professional with any questions or concerns you may have.
• Care4Today® Connect will not identify or alert you to any potential drug-drug interaction. For medical inquires and advice, including those related to drug-drug interactions, please consult your doctor or other qualified healthcare professional.
• The 7-Day Adherence Percentage calculation, available on Care4Today® Connect, is a snapshot of your own self-reporting of adherence to your dosing schedules. The adherence percentage is calculated by the number of doses that have been indicated as taken, divided by the number of doses scheduled over the last 7 days.
• Because the doctor-ordered dosing schedule for a particular medication may require strict adherence, any adherence percentage, even a very high one, may not be relied upon as an indicator that you are taking your medications when and as prescribed. Only your doctor can confirm if you are taking and adhering to your medication as prescribed.
• Medication photos are sourced from a third party database. Care4Today® Connect does not independently validate the accuracy of such photos.
• The reminder functionality available in Care4Today® Connect is populated by information you provide or select in the application about your medications, appointments, and activities. Please ensure that the information you enter into or select in the application is correct and complete to ensure the accuracy of reminders and notifications. The reminders will be delivered at the time you set in the application. If you switch time zones and your phone automatically adjusts to the appropriate time zone or if you manually adjust your phone, the application reminder will be delivered as scheduled.

Privacy

• If you sign up for Care4Today® Connect we intend to send you emails regarding application maintenance and product feedback.
• We may also ask if you want to consent to providing Personal Health Information to your healthcare professional.
• Johnson & Johnson Health and Wellness Solutions, Inc., its affiliates and agents may remove all personal identifiers that you and other users of the application have entered and use non-identifiable information without restriction, including but not limited to sharing such information with third parties for research or commercial purposes. The personal identifiers that will be removed are: name, date of birth, dates, telephone numbers, email address, device identifiers and serial numbers, URL and IP address numbers, and any other unique identifying number, characteristic or code.
